If you are due to travel and would like information about immunisations please download our Travel Clinic form HERE, fill it in and hand it into the surgery at least 6 weeks prior to travelling (at least 8 weeks for back packers). 

The practice nurse will then contact you, advise you if any travel immunisations are required and book an appointment time for you to have these done.
